About MD Custom Rx

At MD Custom Rx our commitment to providing the highest quality compounded prescriptions is demonstrated by holding an accreditation through the Pharmacy Compounding Accreditation Board (PCAB), a division of Accreditation Commission for Health Care (ACHC). We are one of a select few pharmacies in Wisconsin to hold this distinction in non-sterile compounding and we hold it the longest! MD Custom Rx has proudly been accredited since 2009!

When choosing a compounding pharmacy, here are some questions to help ascertain whether the pharmacy in question is devoted to compounding pursuant to the highest quality standards.

Does the pharmacy have PCAB accreditation?

Yes! Working with a compounding pharmacy that has a PCAB accreditation guarantees its quality, giving you peace of mind.

What training does the compounding staff undergo?

Does compounding personnel undergo potency testing on the dosage forms they are validated to compound? Ask to see those test results.

How does the pharmacy ascertain the quality it provides?

The pharmacy should qualify their venders, obtaining and verifying certificates of analysis on all ingredient they use, they should utilize bar code scanning and scale integration, they should perform internal audits on compounding records verifying accuracy, each and every preparation made should meet set quality control/quality assurance parameters.

The Pharmacy Compounding Accreditation Board (PCAB®)

MD Custom Rx is Wisconsin’s oldest PCAB accredited pharmacy, achieving accreditation in 2009!

MD Custom Rx is a PCAB Accredited compounding pharmacy, certified to follow the highest level of prescription compounding standards.

The MD Custom Rx Compounding Pharmacy has been awarded PCAB Accreditation by the Prescription Compounding Accreditation Board. The PCAB Accreditation is awarded only to compounding pharmacies who:

  • Use only chemicals from highly reliable suppliers
  • Obtain regular, specialized training for compounding pharmacists and technicians
  • Maintain pharmacy facilities meeting design and other related standards to ensure pharmacy is clear and free of contamination
  • Meet or Exceed USP <795> standards for prescription compounding
  • Adhere to the PCAB “Principles of Compounding”

For pharmacies to become PCAB Accredited, they must submit documented proof they meet PCAB quality standards, submit their prescription compounding policies and procedures for regular review, and make their compounding pharmacy available to regular physical inspections.

Getting your compounded prescriptions from a PCAB Accredited pharmacy is important because it helps to ensure your prescription will be safe and potent. There are many compounding pharmacies that may not be stocking the highest-grade pharmaceuticals or may use drugs which have passed their expiration date or may not have a proper clean room for sterile compounding. Often, when you come to a compounding pharmacy, it is because conventional medication dosages are not working, and your doctor has decided you might benefit from something different. That’s why it is crucial to KNOW you are receiving medications in the right dose and form from your compounding pharmacy – to take prescription quality issues out of the equation.

MD Custom Rx is one the few compounding pharmacies in Wisconsin to receive PCAB accreditation, and MD Custom Rx offers a much broader range of prescription compounding options than any other Wisconsin compounding pharmacy, as well as a higher level of personalized attention and care.

While all compounding pharmacies are required to meet the requirements set by their respective state boards of pharmacy, PCAB Accredited denotes a more stringent and comprehensive standard which serves as an assurance that our compounding pharmacy has been tested against the profession’s most rigorous standards.

PCAB Accreditation Requires That A Pharmacy

PCAB criteria were established by a standards committee of compounding pharmacists and nationally recognized experts in the compounding pharmacy profession.

To earn our designation as a PCAB Accredited compounding pharmacy, we completed an extensive application and submission process, documenting our education, training, licensures, policies, and procedures. Our documentation was reviewed by PCAB. Next, we had to pass an extensive on-site inspection. Only when this stringent evaluation was completed and we were judged to have met all criteria, did the PCAB officially grant us the PCAB Seal of Accreditation.

We are honored to receive the PCAB Seal of Accreditation. We feel it confirms our commitment to providing safe, personalized solutions that meet the medical needs of our patients – and the needs of the healthcare providers in our community.
